My Healthcare Summary

When you are 18 you will receive a copy of your Health Summary from the Children in Care Health Team

The Health Summary tells you about your health and the health information we hold about you from birth to present day. It is useful as it will hold your NHS number and some important medical facts about you.

If you have not received your health summary, please speak to your Personal Advisor and they will follow this up.
